Transcript Request

To order transcripts or any other student records, you can either fax or mail the request to Suzi Gauthier in Instructional Services.

All requests must be signed. If the student is under 18 years old, the request must be signed by the legal guardian. If the student is 18 years old or over, the request must be signed by the student. Please include the name and address and/or fax number where you want the records sent and which specific records are being requested. Also include a contact number in case there are any questions. You can use the form below to make the request.

We have strict state and federal privacy laws. We will fill requests when the appropriate person signs for them, unless we are sending records directly to another Florida school district. Also, we are unable to provide copies of third-party records. This means we cannot send documentation from other organizations or people (such as outside doctor reports and evaluations or records from other schools). You must contact the third party to request a copy of those records directly.
