Checklist

To assist you in the application process, we provide a checklist of required documents.

  • The most recent IEP (complete copy with all pages, including goals and objectives)
  • Matrix of Services (including cover page and all domains)
  • All Psychological and Educational Testing (or a written statement from the school saying these records do not exist)
  • High School Transcript (for High School Students)
  • Report Cards (for Elementary and Middle School Students)
  • ESOL Information
  • Audiological Testing (graph format)
  • Speech/Language Evaluation
  • Functional Vision Evaluation
  • Ophthalmologist reports
  • Social History
  • FCAT Scores (Florida students only)
  • Behavior/Suspension/Expulsion Records (or a written statement from the school saying there are none of these records)
  • Occupational Therapy Evaluation, CERT and Report (if child is identified OT on the IEP)
  • Physical Therapy Evaluation, CERT and Report (if child is identified PT on the IEP)
  • Birth Certificate
  • Custody Records

Medical Records

  • Immunization Records
  • School Physical (not more than 12 months old)
